Job Responsibilities

Collaborate with the Home Health RN Care Manager and other disciplines to provide skilled care, education, treatment, and emotional support to patients.
Monitor and assess care provided to ensure patients attain their highest level of functioning.
Ensure that the Physician and all care team members are aware of pertinent changes in the patient’s condition and progress toward goals.
Collaborate with the Home Health RN Care Manager to ensure the established plan of care is appropriate and modified based on patient responses to treatment.
Provide skilled nursing care to an assigned caseload of patients.
Monitor the care, treatment, and service provided by other disciplines to ensure patients receive the required care.
Educate patients and their families on proper home health care strategies and procedures.
Instruct and supervise the patient and family in the care of the patient and maintenance of a healthy environment.
Evaluate and revise the plan of care in collaboration with the Home Health RN Care Manager based on patient needs and goals.
Review the plan of care to ensure physician orders are followed and patients receive the necessary care to achieve goals.
Perform skilled assessments and document findings in the OASIS accurately and timely.
Measure wounds weekly and maintain an accurate medication list for each patient.
Communicate regularly with the Home Health RN Care Manager regarding patients, progress toward goals, barriers to care, and outcomes.
Establish positive working relationships with physicians and their staff, keeping them informed of patient problems and progress.
Assess patient outcomes and offer suggestions for improving outcome scores.
Participate in the performance improvement plan and process to ensure positive patient outcomes.
Actively participate in IDT meetings.
Participate in on-call rotation.
Maintain performance, patient and employee satisfaction, and financial standards as outlined in the performance evaluation.
Perform compliance requirements as outlined in the Employee Handbook.
Adhere to the DCH Behavioral Standards.
Use electronic mail, time and attendance software, learning management software, and intranet as required.
Fulfill any additional duties as assigned.

Required Skills

Registered Nurse with a current Alabama license
Skilled nursing care
Patient education

Preferred Skills

1 year of recent home health or acute care nursing experience
Experience in home health care
Extensive clinical knowledge and experience with complex diagnoses
Current BLS certification or ability to obtain within 60 days

Requirements

Experience: Preferred 1 year of recent home health or acute care nursing experience or a combination of both.

Education:

License & Certification:

Registered Nurse license in Alabama
BLS certification
