Job Responsibilities
- Perform visits and complete coordination of client care.
- Act as the point of contact for all disciplines involved in patient care.
- Oversee the frequency of visits for the episode.
- Consult with the physician as needed and provide details about patient care.
- Perform accurate OASIS collection.
- Ensure the medication profile remains current.
- Report lab values to the physician in a timely manner.
- Attend weekly case conferences and monthly case manager meetings.
