Job Responsibilities
- Assist patients in navigating post-acute care to create a positive impact on patient outcomes and referral source satisfaction.
- Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of patient-centered transition plans.
- Promote quality and efficiency in the delivery of post-acute care.
- Represent area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, physician groups, and executive level opportunities.
Required Skills
Core Skills
- Licensed in the state of practice
- Field experience in healthcare
- Knowledge of federal, state, and local laws and regulatory guidelines governing home health and hospice
Preferred Skills
- Registered Nurse or Physical Therapist degree
- Three years of field experience
- Experience in home health or healthcare sales
- Basic technology skills including mobile device operation
Requirements
Experience: Two years' demonstrated field experience
Education: Graduate of an approved school of nursing, therapy, or social work
License & Certification:
- Valid state driver license
- Automobile liability insurance
