Job Responsibilities

Rounding and managing the clinical care of the population at the assigned skilled nursing facilities (SNF).
Prescribing medications and treatments for residents at SNFs.
Providing high quality, patient-centered, comprehensive primary care services to patients in SNFs.
Timely documentation of all patient care encounters.
Available for phone/secure messaging consultations with facility nursing staff.
Completed documentation in proprietary electronic health record (EHR) system, ChartEasy™.
On-site attendance and punctuality for scheduled shifts.
