Job Responsibilities
- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location.
- Conduct and delegate assessments and reassessments of patients, including care plan updates.
- Manage the assignment of caregivers.
- Ensure the delivery of care to all patients served by the location.
- Receive and review case referrals to determine home health or hospice needs.
- Ensure patients meet admission criteria and decide to admit patients to service.
- Assign appropriate clinicians to a case as needed.
- Instruct and guide clinicians to promote effective performance and quality home care services.
- Assist clinicians in establishing therapeutic goals and developing patient Plan of Care (POC).
- Monitor cases to ensure compliance with documentation requirements and regulatory agencies.
- Coordinate communication between team members, attending physicians, and caregivers.
- Work with the Branch Director and Finance Department to establish revenue and budget goals.
- Participate in sales and marketing initiatives.
- Supervise all clinical employees assigned to a specific location.
- Carry out supervisory responsibilities according to company policies and procedures.
- Handle employee corrective action and discipline issues fairly and objectively.
- Participate in the interviewing, hiring, training, and development of direct care clinicians.
- Evaluate staff performance relative to job goals and requirements.
- Coach staff and recommend in-service education programs as needed.
- Assess staff education needs based on clinical documentation review.
- Create and conduct regular staff education as needed.
- Analyze situations, identify problems, and evaluate alternative courses of action.
- Review clinical staff documentation for appropriateness of care and documentation requirements.
- Responsible for QA/PI activities and performance review data tracking.
- Provide direct patient care on an infrequent basis in emergencies.
- Act as Branch Director in their absence.
Required Skills
Core Skills
- Current state license as a Registered Nurse
- Proof of current CPR
- Experience with OASIS
- Experience with Homecare HomeBase (HCHB)
- Valid driver’s license and reliable transportation
Preferred Skills
- Two years as a Registered Nurse
- At least one year of management experience in home care, hospice or equivalent environment
Requirements
Experience: 5+ years
Education: Graduate of an accredited School of Nursing
License & Certification:
- Registered Nurse license
- Current CPR certification
