Job Title: Licensed Practical Nurse - Ambulatory Care / Patient Aligned Care Team

Position: LPN at the Corporal Michael J Crescenz Department of Veterans Affairs Medical Center

Job Responsibilities

  • Provide care of patients/residents and their environment to optimize patient/resident outcomes.
  • Practice within the scope and licensure of an LPN/LVN in the nursing unit, clinic, or area of assignment.
  • Provide care and services to various patient populations as needed.
  • Perform reporting, communication, and documentation in the computerized patient record system.
  • Demonstrate competency with equipment and technical standards.
  • Maintain reusable medical equipment according to standard operating procedures.
  • Participate in Medical Center activities and promote customer relationships.
  • Ensure patient/resident safety standards including privacy and psychological safety.
  • Establish working relationships with team members and promote safety.
  • Maintain licensure, BCLS, and complete required education and training.
  • Administer medications within the scope of licensure.
  • Support the Registered Nurse and/or provider as allowed.
  • Participate in patient education programs.
  • Provide care consistent with Infection Control Standards.
  • Recognize and respond appropriately to medical emergencies.

Experience: No specific years required, but preferred experience includes 2 years in medical surgical and primary care settings.

Education: Graduate of an approved school of practical or vocational nursing.

License & Certification:

  • Active, full, current, and unrestricted LPN licensure
