Job Responsibilities
- Provide holistic patient care in a team-based hospice setting.
- Coordinate care and education for patients.
- Assess patients and develop/update care plans.
- Ensure accuracy of documentation.
- Coordinate clinical, psycho-social, and spiritual services.
- Collaborate with the Interdisciplinary Team.
- Explain hospice and Medicare benefits to patients.
- Obtain consent for treatment.
- Effectively communicate patient needs.
- Supervise hospice aides and nurses.
- Adjust assignments of hospice aides and nurses as needed.
- Provide emergency on-call support.
- Maintain compliance with regulatory standards.
- Promote positive working relationships within the team.
