Job Responsibilities

Facilitate the patient's progress through the acute episode of care efficiently and cost-effectively.
Monitor the level of care and initiate evidence-based protocols and order sets according to the patient's admission diagnosis.
Influence the progression of care and facilitate the patient's navigation through the acute care episode.
Propose an appropriate transition plan within 48 hours of admission.
Serve as an advocate for the patient and family throughout the acute episode of care.

Required Skills

Core Skills

Knowledge of theories, principles, and concepts related to nursing practice.
Strong leadership capabilities.

Preferred Skills

Knowledge of InterQual criteria.

Requirements

Experience: 1 year experience in acute care Case Management setting or Utilization Management at an acute care facility and/or insurance company.

Education: Bachelor's Degree in Nursing (BSN) or combination of education and experience with commitment to obtain BSN within 2 years from date of hire.

License & Certification:

PA Registered Nurse License (RN-LIC)
Certified Case Manager (CCM) (Preferred)
