Job Responsibilities

  • Identifies and implements placement of services to enhance patient outcomes.
  • Highlights patients at high to moderate risk for readmission.
  • Serves as the contact for patients, families, and community liaisons for post-acute services coordination.
  • Runs daily readmission reports.
  • Provides support and education to patients and families regarding the discharge process.
  • Begins the coaching relationship with patients.
  • Assists with referrals based on patient and family choice.
  • Promotes effective utilization of healthcare resources for placement in post-discharge care.
  • Facilitates updates to patients' discharge plans.
  • Works collaboratively with the interdisciplinary healthcare team for 30 days post-discharge.
  • Collaborates with physicians, patients, and families in after-care planning.
  • Documents all discharge planning activities in Cerner.
  • Monitors hospital patient satisfaction and physician satisfaction scores.
  • Contacts payers to assess benefit coverage for post-discharge care.
  • Facilitates and expedites discharge when patients are ready for lower levels of care.
  • Attends readmission meetings and provides feedback.

Requirements

Experience: 1-3 years case management experience preferred; Two years hospital-based clinical experience.

Education: Associates Degree in Nursing required; Bachelor's Degree in Nursing preferred.

License & Certification:

  • Current RN license to practice in NC or multi-state (compact) license
  • NIH Stroke Scale certification required
