Job Responsibilities
- Manage and coordinate the patient care transitions program.
- Ensure the patient's progression through the care continuum to achieve designed clinical and financial outcomes.
- Participate in the system development and implementation of integration of care transitions.
- Collaborate with the multidisciplinary team in ambulatory, post-acute providers, and health plans.
- Serve as a mentor by role modeling a professional practice consistent with organizational goals.
- Develop and manage the departmental budget.
- Ensure compliance with federal, state, and regulatory requirements.
Required Skills
Core Skills
- Leadership experience
- RN Care Management experience
- Private Duty Nursing
- High-Risk Pediatric experience
- Knowledge of DMAS, EPSDT, NCQA standards, LTSS, and CCC Plus
Preferred Skills
- Specialty certification (ACM, CCM, CCCTM, CMAC, or RN-BC)
- Experience with data analysis
- Experience with Epic and JIVA
- Case management and care coordination
Requirements
Experience: One year of Leadership experience and three years of RN Care Management experience required.
Education: BSN Required, Master's degree preferred.
License & Certification:
- Active RN license
- Specialty certification required within one year of eligibility for Integrated Care Management departments
