Job Responsibilities
- Provide care management for patients with chronic illness and/or multiple co-morbidities.
- Utilize a set of activities intended to improve patient care and reduce the need for medical services.
- Help patients and caregivers effectively manage their health conditions and related psychosocial problems.
- Improve patients' functional health status and enhance coordination of care.
- Eliminate duplication of services in a cost-effective manner.
- Support patients in achieving an optimal level of wellness as defined by the patient.
- Assist patients to gain self-efficacy and management skills.
- Achieve optimum functional health status and quality of life.
- Collaborate with patients, families, staff, and systems to achieve high quality, evidence-based, cost-effective, and patient-focused outcomes.
Required Skills
Core Skills
- Current Washington State licensure as an RN
- Strong patient assessment skills
- Ability to work collaboratively in a team setting
- Current working knowledge of electronic medical records
Preferred Skills
- CPR certification (required every two years)
- Bachelor of Science in Nursing
- American Heart Association Resuscitation Quality Improvement competency certification
