Job Responsibilities

  • Provide nursing care to a defined group of patients adhering to Kaiser Foundation Health Plan of Washington nursing standards.
  • Coordinate and facilitate health care services for patients in various segments of the health care delivery system.
  • Oversee and participate in patient assessment and care plan formulation.
  • Provide ongoing assessment of nursing care needs using the nursing process.
  • Develop a written plan of care for patient needs.
  • Assist patients and families in developing health care goals.
  • Determine priorities of patient care based on essential needs and available resources.
  • Provide safe and competent nursing interventions.
  • Communicate with patients and family about nursing interventions.
  • Provide telephone consultation to patients and families.
  • Evaluate patient responses to care and modify plans in collaboration with the health care team.
  • Demonstrate understanding of aging effects by assessing care needs for various age populations.
  • Communicate effectively with the designated age populations served.
  • Assess and initiate nursing admission database and discharge planning in collaboration with inpatient staff and physicians.
  • Identify and address patients and families' psychosocial, cultural, and emotional needs.
  • Develop educational components for each nursing care plan based on assessment of educational needs.
  • Encourage patient compliance with therapeutic and medical regimens.
  • Integrate health promotion and preventive aspects of care.
  • Establish a safe environment and adhere to safety and infection control standards.
  • Participate in continued education programs to meet learning needs.
  • Participate in professional activities, nursing research, and quality assurance activities.

Requirements

Experience: Nursing experience within the last five years

Education: Associates degree in Nursing from an accredited program

License & Certification:

  • Registered Nurse License (Washington) or Compact License required at hire
  • Advanced Cardiac Life Support required at hire
  • Basic Life Support required at hire
  • Pediatric Advanced Life Support required at hire
