Perform comprehensive patient assessments and collaborate with facility staff on preventive services and early changes in condition recognition.
Manage acute and chronic conditions, including medication adjustments.
Order and interpret diagnostic tests.
Develop, implement, and modify treatment plans as needed.
Participate in quality assurance and improvement activities.
Provide clinical education and training for patients, families, and facility staff.
Adhere to state-specific NP scope of practice requirements.
Demonstrates knowledge of etiologies, risk factors, pathophysiology, and presentation of acute and chronic conditions.

Master’s degree in Nursing or RN license
Advanced Practice Nursing license
NP Board Certification
Strong communication, organizational, and customer service skills
Ability to handle a high volume of patients with skill and confidence
Knowledge and use of an Electronic Health Record
Ability to work with confidential information including HIPAA protected information
Driving capability with a current state driver's license and proof of insurance

Credentialed with insurance companies
1 to 2 years' NP experience
Experience with adult and geriatric populations and population health management
Knowledge of Skilled Nursing/Transition Rehabilitation Centers

Experience: 1 to 2 years' NP experience preferred

Education: Master’s degree in Nursing or RN license
