Job Title: Nurse Family Partnership Nurse Home Visitor (Bilingual)
Position: Nurse providing in-person and virtual nursing care.
Job Type: Full Time

Job Responsibilities
- Assess patients' well-being during home visits.
- Maintain accurate records of the services provided.
- Collaborate with other care providers and physicians.
- Monitor and record patients' vital signs, when necessary.
- Develop and maintain patient care plans.
- Document visits and submit information within the specified time frame.
- Perform other related duties as assigned.
Required Skills
Core Skills
- Registered Nurse (RN)
- Associate Degree in Nursing (ADN/ASN)
- Current BLS certification
- Excellent verbal and written communication skills
- Excellent interpersonal and customer service skills
- Excellent organizational skills and attention to detail
- Excellent time management skills
- Strong analytical and problem-solving skills
- Ability to prioritize tasks
- Proficient with Microsoft Office Suite or related software
Preferred Skills
- Bachelor of Science in Nursing (BSN)
- CLC/IBCLC certification
- CPR certification
- Bilingual
Requirements
Experience: Experience in Maternal Child Health, Pediatric Health, Mental Health, and/or case management is a plus.
Education: Bachelor of Science in Nursing; Registered Nurse who will complete BSN within one year.
License & Certification:
- Registered Nurse (RN)
- Current BLS certification
