Job Title: Hospice Resource Nurse
Position: Admissions Team RN
Job Type: Full Time
Pay: $85,000-$92,000 + Bonus opportunities
Shift: 7 days on, 7 days off, 10 AM - 6 PM

Job Responsibilities
- Provide care and services for a group of hospice patients.
- Visit patients where they reside, including private homes, assisted living facilities, and hospitals.
- Work after hours on-call during the scheduled 7 days/nights on and 7 nights/days off.
- Collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
- Manage documentation and communication through phone, email, and written formats.
- Attend required meetings and respond to urgent needs by prioritizing and triaging multiple concerns.
- Learn and utilize an electronic medical records system.
- Drive reliably across the service area with valid driver's license and auto insurance.
Required Skills
Core Skills
- Registered Nurse (RN) licensure in good standing
- One year of experience under license
- Time management skills
- Ability to work independently and in a team environment
- Understanding of hospice rules and regulations
Preferred Skills
- Hospice experience, particularly in admissions
- On-call experience
