Assists patients in navigating post-acute care to positively impact patient outcomes and referral source satisfaction.
Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and metrics in developing patient-centered transition plans.
Promotes quality and efficiency in delivering post-acute care.
Represents area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and physician groups.

Demonstrated field experience in healthcare
Understanding of federal, state, and local laws governing Medicare certified home health and hospice
Basic technology skills, including mobile device operation

Experience: Two years' demonstrated field experience

Education: Graduate of an approved school of nursing, therapy or social work
