Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts systematic, dynamic assessments including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Forms a plan of care.
Implements nursing care according to the care plan.
Ensures patient safety and medication administration.
Provides ordered treatments and patient/family education.
Documents care in the healthcare record.
Continuously evaluates patient status and effectiveness of nursing care.
Manages and corrects the sequence, timing, and effectiveness of patient care from admission to discharge.

Education: Graduation from an accredited School of Nursing; Preferred: Bachelors of Science in Nursing
