Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial assessments and re-assessments based on patient condition.
- Identifies problems related to the patient’s response to actual or potential health conditions.
- Sets measurable and achievable goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates and documents a plan of care.
- Ensures patient safety through medication administration and provision of ordered treatments.
- Educates patient and family regarding care.
- Continuously evaluates patient status and the effectiveness of nursing care.
- Coordinates care from admission to discharge, managing timing and effectiveness.
- Delegates and supervises care tasks as needed.
- Prioritizes patient care based on clinical judgment.
Required Skills

- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) certification

- Advanced Cardiac Life Support (ACLS)
- Pediatric Advanced Life Support (PALS)

Requirements
Experience:
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid Arizona Nursing License
- Basic Life Support (BLS)
- Advanced Cardiac Life Support (ACLS) - preferred
