Assist in managing activities of Transitional Care Management (TCM) nurse activities.
Facilitate the TCM program and supervise staff in discharge planning and post-acute care services.
Collaborate with Case Management directors and senior leadership to support and implement TCM activities.
Coordinate care and services for selected patient populations.
Design plans of care to avoid patterns of frequent acute hospital or emergency department use.
Complete established competencies for the position within the introductory period.
Perform other related duties as assigned.
