Job Responsibilities
- Coordinate the interdisciplinary assessment process for assigned residents of the facility.
- Coordinate development of and ensure completion of Comprehensive Care Plan in accordance with company, state and federal guidelines.
- Monitor residents to ensure that the individual resident Care Plans are followed and updated as necessary.
- Bring any variances noted to the attention of the Director of Nursing and Care Plan Team.
