Job Responsibilities

Provide skilled nursing services to patients struggling with substance use disorders.
Manage detoxification, ongoing assessment, evaluations, triage, harm reduction teaching, stabilization, and monitoring of clients/patients.
Develop goal-oriented treatment plans with clients/patients.
Collaborate with primary care providers, integrated treatment team, and community providers.
Facilitate and schedule intakes, discharges, and transfers to residential or outpatient programs as necessary.
Provide nursing care to patients, including medication monitoring and adjustments within limits prescribed by medical direction or physician order.
Assess and develop appropriate goal-oriented treatment plans based on individual client/patient needs.
Ongoing management of patients receiving medications for addiction.
Act as a liaison between client and community caregivers.
Regularly evaluate client/patient progress and provide crisis intervention as appropriate.
Submit timely clinical documentation.
Coordinate services with the client, family, and all providers involved in the treatment plan.
Determine if a medical emergency exists and take appropriate action.
Provide advocacy to ensure clients/patients understand their rights.
Educate and support clients/patients accessing opioid treatment programs and promote wellness and recovery.
Assess clients/patients systematically for signs and symptoms.
Involve clients/patients and significant others in data collection when applicable.
Use clear and effective communication in all documentation and patient care data sharing.
Provide follow-up care in person and via telephone as needed.
Maintain up-to-date knowledge on current addiction treatment trends and local resources.
Participate in cross-functional teams and performance improvement initiatives.
Perform other related duties as required.

Required Skills

Proficiency in detoxification and identifying signs of withdrawal.
Ability to take and record vital signs.
Competence in treatment of addiction, motivational counseling techniques.
Excellent customer service and communication skills.
Strong judgment and problem-solving skills.

Preferred Skills

Basic PC skills including Microsoft Office Suite.
Cultural sensitivity and the ability to work with diverse groups.
Additional language abilities.

Requirements

Experience: At least one year of professional nursing experience, preferably in detox unit, inpatient, partial hospitalization, or outpatient setting.

Education: Must be a Registered Nurse (RN) licensed in Texas.
