Job Responsibilities
- Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME
- Develops, coordinates, and follows the patient's individualized Plan of Care
- Coordinate with team members/physicians and participate in staff and Interdisciplinary Team meetings
- Appropriately identify and utilize resources: Dietary, Pharmacy, Supplies, Home Medical Equipment, etc.
- Serve as a patient and family advocate to ensure delivery of the highest quality of care, providing education to patients, families and caregivers
- Complete documentation in a timely manner
- Communicate effectively with coworkers and patients/family
- Participate in bereavement support
