Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessments, re-assessments, and focused assessments based on patient condition, setting and population.
Identifies problems regarding patient response to actual or potential health conditions or needs.
Plans and sets measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care and documents it in the patient’s care plan.
Implements nursing care according to the care plan ensuring continuity of care during hospitalization.
Documents care in the patient’s record.
Evaluates patient’s status and effectiveness of nursing care and modifies care plan as needed.
Coordinates care from admission to discharge.
