Deliver optimal patient care through effective clinical practice.
Educate patients, their families, clinical staff, and others.
Consult with other healthcare providers.
Maintain current knowledge regarding options for optimal patient care and outcomes.
Perform Adult-Geriatric patient assessments to obtain accurate clinical information.
Order tests and exams to ensure complete care for patients with chronic illness.
Diagnose patient needs and determine a plan of care, including patient goals and palliative care needs.
Interpret patient care tests and exams to guide care.
Provide treatment and oversee treatment provided by others.
Document clinical care appropriately and timely, and ensure accurate coding and billing.
Conduct and coordinate Geri-centric education to increase health literacy.
Train other staff in areas of clinical expertise and provide teaching of students.
Participate in continuing medical education to ensure clinical knowledge and patient care skills.
Coordinate and collaborate patient care efforts with other providers.
Communicate clinical information effectively to other providers.
Participate in research and utilize evidence-based practices to promote quality patient care.

Experience: At least two years working as an APP in Adult Medicine.

Education: Master's degree or higher.
