Job Responsibilities

Develops and implements appropriate support plans to meet specific objectives.
Responds to negative trends in performance metrics by assisting local operations and nursing leadership in developing action plans.
Mentors and directs operations to ensure service quality is consistent with applicable laws and company policies.
Collaborates with operations management to help meet business objectives and support health and wellness.
Mentors local operations and clinical leadership on the clinical reimbursement process; provides education to improve documentation.
Provides direction for clinical process improvement methods and guides staff in implementation.
Educates on company policy, clinical processes, and procedures for leadership and direct care/support staff.
Interviews and evaluates candidates for local operations and clinical leadership positions.
Facilitates implementation of company initiatives and changes in policies and procedures.
Collaborates with corporate clinical and quality departments for consistent field direction.
Proactively identifies care issues and collaborates with local leadership for performance improvement.
Ensures there is a nurse on call after hours for all homes.
Reviews incident reports and coordinates completion of mortality reviews.
Provides consultation services to nursing personnel on medication administration and wellness.
Acts as primary resource for nursing personnel in assigned state for training and mentorship.
Oversees application of QuickMar and EHR.
Participates in investigations as requested and attends company investigator training.
Facilitates communication between operations and nursing staff and medical providers.
Works with nursing personnel on survey preparation and staffing concerns.
Provides survey management support and coordinates corrective action plans.
Monitors nursing reports to ensure timely medical follow-up.
Consults on care of medically fragile individuals and complex care needs.
Facilitates compliance with financial obligations regarding staffing needs and medication orders.
Provides feedback to operations leadership regarding nursing service changes.
Acts as resource for admissions/placements to ensure appropriateness.
Ensures active Infection Control program and adequate PPE resources.
Completes performance evaluations on direct reports as outlined by company procedures.
Participates in special projects and performs other duties as assigned.

Requirements

Experience: At least 2 years of nursing management/supervisory experience, including 1 year of IDD or SNF related experience.

Education:

License & Certification:

Registered Nurse (RN) license, current and in good standing
Valid driver’s license from state of residence

Schedule:

50 – 75% in-state travel based on needs of individuals served and staff training.
