Job Responsibilities
- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for the specific location managed.
- Conduct/delegate assessments and reassessments of patients, including care plan updates.
- Manage the assignment of caregivers and oversee care delivery to patients.
- Receive case referrals and determine home health or hospice needs.
- Ensure patients meet admission criteria and make decisions to admit patients to service.
- Assign appropriate clinicians to cases as needed.
- Instruct and guide clinicians for effective performance and delivery of quality home care services.
- Monitor cases for documentation compliance with regulatory agencies and third-party payer requirements.
- Ensure timely completion of audits/billing in compliance with Medicare regulations.
- Coordinate communication between team members, attending physicians, and caregivers.
- Work with the Branch Director and Finance Department to set location revenue and budget goals.
- Participate in sales and marketing initiatives.
- Supervise clinical employees at the specific location.
- Handle employee corrective actions and discipline issues objectively.
- Participate in the hiring, training, and development of direct care clinicians.
- Evaluate clinician performance and recommend in-service education programs.
- Conduct staff education based on clinical documentation reviews and feedback.
- Analyze situations and evaluate alternative courses of action using Performance Improvement principles.
- Review Case Manager and clinical staff documentation for appropriateness of care and compliance.
- Responsible for QA/PI activities and work with Utilization Review staff on data tracking.
- Participate in performance improvement activities and maintain ongoing clinical knowledge.
- Maintain relationships with referral/community sources and participate in professional organizations.
- Provide direct patient care in emergencies and act as Branch Director in their absence.
- Interpret Company standards and ensure caregiver documentation meets internal standards.
Required Skills
Core Skills
- Current state license as a Registered Nurse
- Management experience in home care or hospice
- Home health experience
- OASIS experience
Preferred Skills
- Management and people leadership experience
- OASIS certification (HCS-O, COQS, and/or COS-C)
- Homecare Homebase (HCHB) experience
- CMS PDGM billing knowledge and/or experience
Requirements
Experience: Two years as a Registered Nurse with at least one year of management experience in home care or hospice environment.
Education: Graduate of an accredited School of Nursing.
License & Certification:
- Registered Nurse license
- CPR certification
- Valid driver's license with auto insurance
