Job Title: RN Clinical Manager, Home Health
Position: RN Clinical Manager overseeing home health services
Job Type: Full-Time
Pay: $77,200 - $106,200 per year, eligible for annual incentive bonus
Shift: M-F, 8 AM - 5 PM ET, with rotating on-call shift

Job Responsibilities
- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location.
- Conduct and delegate the assessment and reassessment of patients, including updating care plans.
- Manage assignment of caregivers and oversee delivery of care to all patients.
- Receive case referrals and review patient information to determine home health or hospice needs.
- Ensure patients meet admission criteria and make decisions to admit patients to service.
- Instruct and guide clinicians to promote effective performance and delivery of quality home care services.
- Assist clinicians in establishing immediate and long-term therapeutic goals and developing patient Plan of Care.
- Monitor cases to ensure documentation compliance with regulatory agencies and payer requirements.
- Coordinate communication among team members, physicians, and caregivers for care appropriateness.
- Work with Branch Director and Finance Department to establish revenue and budget goals.
- Participate in sales and marketing initiatives.
- Supervise all clinical employees at the location and handle employee corrective actions.
- Participate in interviewing, hiring, training, and development of direct care clinicians.
- Evaluate clinician performance relative to job goals and recommend education programs.
- Assess staff education needs and conduct regular staff education as necessary.
- Analyze situations and identify problems to evaluate alternative courses of action.
- Review clinical staff documentation for appropriateness of care and delivery requirements.
- Manage QA/PI activities and track performance for review and outcomes of care analysis.
- Provide direct patient care during emergencies only.
- Act as Branch Director in their absence.
- Ensure compliance with external regulatory authorities and internal standards.
Required Skills
Core Skills
- Current state license as a Registered Nurse
- Management experience in home care or hospice environment
- CPR certification
- Case management skills
- Document compliance knowledge
Preferred Skills
- Home health experience
- OASIS experience and certification (HCS-O, COQS, and/or COS-C)
- Homecare Homebase (HCHB) experience
- Knowledge of CMS PDGM billing
Requirements
Experience: Two years as a Registered Nurse with at least one year of management experience
Education: Graduate of an accredited School of Nursing
License & Certification:
- Current state license as a Registered Nurse
- Proof of current CPR
Schedule:
- Normal Hours of Operation: M-F / 8 AM - 5 PM (ET)
- Rotating on-call shift required
