Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring.
Identify opportunities for CM and integrate local CM processes.
Participate in all phases of the Case Management Program (CMP) ensuring that the CMP meets established standards of care.
Develop and implement local strategies using inpatient, outpatient, onsite, and telephonic CM.
Implement tools to support case management, such as patient identification and assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources.
Integrate CM and utilization management (UM), along with nursing and social work case management.
Maintain liaison with community agencies and organizations.
Accurately collect and document patient care data.
Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care.
Establish mechanisms for proper implementation of patient treatment plans and follow-up post discharge.
Provide health care instruction to patients and/or caregivers based on identified learning needs.

Experience: One year of experience in nursing as identified after graduation.

Education: Associates Degree of Nursing from an accredited college or university.

License & Certification:

Current, full, active, and unrestricted license to practice as a Registered Nurse

Schedule:

Normal duty schedule either 0700-1600 or 0730-1630 Monday-Friday or Flex shifts
Flex shifts must be approved by the Government Supervisor and the COR and can consist of 8, 9, or 10 hour shifts not exceeding 80 hours in a two week pay period.
