Job Responsibilities
- Provide nursing services independently and as part of a specialty medical team.
- Administer medications and injections to patients as directed by the providers.
- Conduct health assessments, provide brief interventions, and triage patients.
- Prioritize individuals with high medical and behavioral health needs for intensive visits.
- Assess patient learning needs and develop and implement patient education.
- Respond to immediate survival needs as prioritized by clients.
- Provide medical oversight of Community Health Worker and Behavioral Health Case Manager.
- Link and refer patients to community services as needed.
- Represent TVHC at county and community meetings.
- Participate in multidisciplinary staff meetings and program development processes.
- Maintain confidentiality regarding privileged patient information.
- Provide outreach, engagement, and health services in encampments and service locations.
- Screen and identify individuals with medical and behavioral needs.
Required Skills
Core Skills
- Current California RN license
- Bilingual in English, Spanish, or other languages
- Excellent communication and documentation skills
- Crisis intervention and de-escalation techniques
- Culturally competent care for diverse populations
Preferred Skills
- Two years of nursing experience serving patients with complex needs
- Experience working with unsheltered populations
- Experience with quality improvement processes
Requirements
Experience: Preferably, two years of nursing experience serving patients with complex needs.
Education:
License & Certification:
- Current, valid California RN license
- Current, Valid Cardio Pulmonary Resuscitation Card (CPR)
- Valid CA driver’s license and insurance
