Job Responsibilities
- Supervision and coordination of clinical services.
- Direct provisions of nursing care to patients in their homes as prescribed by the physician.
- Provide clinical services within the scope of practice as defined by state laws governing nursing.
- Receive referrals and ensure appropriate clinician assignments.
- Evaluate patient orders and plot start of care visits.
- Coordinate determination of patient home health benefits and ongoing insurance approvals.
- Ensure patient needs are continually assessed and care rendered meets home health eligibility criteria.
- Review assessments and plans of care daily per assigned workflow.
- Consult clinicians with recommendations, as appropriate.
Required Skills

- Current RN licensure in state of practice
- Current CPR certification
- Driver's License and vehicle insurance
- Ability to access a dependable vehicle or public transportation
