Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial, re-assessment, and focused assessments based on patient condition, setting, and population.
- Identifies problems and sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Forms a plan of care and ensures continuity of care during hospitalization and in preparation for discharge.
- Ensures patient safety and provides medication administration, ordered treatments, patient/family education, and nursing care.
- Documents care in the healthcare record.
- Continuously evaluates patient’s status and the effectiveness of nursing care, modifying care plans as needed.
- Engages in care coordination actions with patients, families, and/or healthcare team members.
Required Skills
Core Skills
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) certification
Preferred Skills
- Advanced Cardiac Life Support (ACLS) certification
- Pediatric Advanced Life Support (PALS) certification
- Neonatal Resuscitation Program (NRP) certification
- Bachelor of Science in Nursing
Requirements

Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS)
