Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessment, re-assessment, and focused assessments based on patient condition, setting, and population.
Identifies problems regarding the patient’s response to actual or potential health conditions or needs.
Plans and sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Implements nursing care according to the care plan to ensure continuity of care.
Documents care in the patient’s record.
Ensures patient safety.
Administers medications and provisions of ordered treatments.
Educates patient/family.
Evaluates both the patient’s status and the effectiveness of the nursing care continuously.
Manages care coordination with patients, families, and/or members of the healthcare team.
Delegates and supervises care as needed.
Prioritizes care tasks.
