Job Responsibilities

Perform assessment and plan patient care.
Evaluate the patient assessment, planning, and intervention process.
Demonstrate professional and personal development.
Deliver direct patient care appropriate to the age of the patient served.
Assess and diagnose wounds, ostomies, and continence disorders.
Develop and implement treatment plans.
Monitor and evaluate patient progress.
Provide patient education on self-care and disease management.
Collaborate with other healthcare professionals to ensure holistic care.
Stay up-to-date with the latest research and technology in wound, ostomy, and continence care.
Document patient progress and treatment plans accurately and thoroughly.
Provide emotional support to patients and their families.
Manage and maintain specialized equipment and supplies.
Advocate for the needs and rights of patients with wounds, ostomy, and continence disorders.
Manage complex wounds, ostomies, and continence issues.

Required Skills

Core Skills

Ability to work independently.
Addresses wound care consults timely.
Ability to teach staff on wound care, staging, and documentation.
Professionally interact with patients.
Ability to teach patients how to manage their wound care.
Facilitates Pressure Injury monthly meeting.
Accurately assess patient skin integrity.
Prevent skin breakdown in hospitalized patients.
Knowledge of Microsoft Office (Word, Excel, PowerPoint).

Preferred Skills

Wound Care Certification.
Two years of experience in wound, ostomy, and continence care.
Experience with Basic Life Support (BLS).

Requirements

Experience: One to two years of related experience and/or training preferred.

Education: Graduate of a Program in Discipline; Bachelor's Degree preferred.

License & Certification:

Current licensure by WV State Board of Nursing.
Basic Life Support (BLS) - to be obtained within 6 months.
