Job Responsibilities
- Perform assessment and plan patient care.
- Evaluate the patient assessment, planning, and intervention process.
- Demonstrate professional and personal development.
- Deliver direct patient care appropriate to the age of the patient served.
- Assess and diagnose wounds, ostomies, and continence disorders.
- Develop and implement treatment plans.
- Monitor and evaluate patient progress.
- Provide patient education on self-care and disease management.
- Collaborate with other healthcare professionals to ensure holistic care.
- Stay up-to-date with the latest research and technology in wound, ostomy, and continence care.
- Document patient progress and treatment plans accurately and thoroughly.
- Provide emotional support to patients and their families.
- Manage and maintain specialized equipment and supplies.
- Advocate for the needs and rights of patients with wounds, ostomy, and continence disorders.
- Manage complex wounds, ostomies, and continence issues.
Required Skills
Core Skills
- Ability to work independently.
- Addresses wound care consults timely.
- Ability to teach staff on wound care, staging, and documentation.
- Professionally interact with patients.
- Ability to teach patients how to manage their wound care.
- Facilitates Pressure Injury monthly meeting.
- Accurately assess patient skin integrity.
- Prevent skin breakdown in hospitalized patients.
- Knowledge of Microsoft Office (Word, Excel, PowerPoint).
Preferred Skills
- Wound Care Certification.
- Two years of experience in wound, ostomy, and continence care.
- Experience with Basic Life Support (BLS).
Requirements
Experience: One to two years of related experience and/or training preferred.
Education: Graduate of a Program in Discipline; Bachelor's Degree preferred.
License & Certification:
- Current licensure by WV State Board of Nursing.
- Basic Life Support (BLS) - to be obtained within 6 months.
