Job Responsibilities
- Performs assessment and plans patient care.
- Evaluates the patient assessment, planning and intervention process.
- Demonstrates professional and personal development.
- Delivers direct patient care appropriate to the age of patient served.
- Assesses and diagnoses wounds, ostomies and continence disorders.
- Develops and implements treatment plans.
- Monitors and evaluates patient progress.
- Provides patient education on self-care and disease management.
- Collaborates with other healthcare professionals to ensure holistic care.
- Stays up-to-date with the latest research and technology in wound, ostomy and continence care.
- Documents patient progress and treatment plans accurately and thoroughly.
- Provides emotional support to patients and their families.
- Manages and maintains specialized equipment and supplies.
- Advocates for the needs and rights of patients with wounds, ostomy and continence disorders.
- Manages complex wounds, ostomies and continence issues.
Required Skills
Core Skills
- Must be able to work independently.
- Addresses wound care consults timely.
- Ability to teach staff on wound care, staging, & documentation.
- Professionally interacts with patients.
- Ability to teach patients how to manage their wound care.
- Accurately assesses patient skin integrity.
- Prevent skin breakdown in hospitalized patients.
- Accurately stages and assesses wounds.
- Microsoft Office skills - Word, Excel, PowerPoint.
Preferred Skills
- Wound Care Certification preferred.
- Two years of experience in wound, ostomy and continence care required.
Requirements
Experience: One to two years related experience and/or training preferred.
Education: Graduate of a Program in Discipline - Required, Bachelor's Degree preferred.
License & Certification:
- Current licensure by WV State Board of Nursing.
- Basic Life Support (BLS) - to be obtained within 6 months.
