Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts systematic assessments, including physiological and psychological factors.
Identifies patient problems and needs through clinical judgment.
Sets measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Implements nursing care according to the care plan.
Ensures patient safety and medication administration.
Provides ordered treatments and family education.
Documents care in the healthcare record.
Continuously evaluates the patient's status and nursing care effectiveness.
Coordinates care from admission to discharge and manages care sequences.
