Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial assessment, re-assessment, and focused assessment based on patient condition.
- Identifies problems based on clinical judgment about patient’s response to health conditions or needs.
- Plans measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates a plan of care.
- Implements nursing care according to the care plan to ensure continuity during hospitalization and discharge.
- Documents care in the patient’s record, ensuring patient safety and appropriate medication administration.
- Provides patient/family education and nursing care.
- Continuously evaluates patient’s status and effectiveness of nursing care, modifying care plan as needed.
- Coordinates actions with patients, families, and healthcare team members to manage patient care.
- Delegates and supervises nursing tasks.
- Prioritizes care based on patient needs.
Required Skills
Core Skills
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) certification
Preferred Skills
- Advanced Cardiac Life Support (ACLS) certification
- Pediatric Advanced Life Support (PALS)
- Neonatal Resuscitation Program (NRP)
Requirements
Experience: None
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) certification
