Job Responsibilities
- Perform visits and complete coordination of client care.
- Act as the point of contact for all disciplines involved in providing care to patients.
- Oversee the frequency of visits for the episode.
- Consult with the physician and the office about patient care as needed.
- Perform accurate OASIS collection.
- Ensure the medication profile remains current.
- Verify lab values have been reported to the physician in a timely manner.
- Attend weekly case conferences and monthly case manager meetings.

- Experience with OASIS collection
Requirements
Experience: A minimum of one year of clinical experience is preferred.
Education:
License & Certification:
- Registered Nurse (RN) license in the state of employment
- Valid state driver's license
- Automobile liability insurance
