Perform visits and coordinate client care
Serve as the point of contact for all disciplines involved with providing care to patients
Oversee the frequency of visits for the episode
Consult with the physician and the office regarding patient care details
Perform accurate OASIS collection
Ensure medication profile remains current
Ensure lab values are reported to the physician timely
Attend weekly case conferences
Participate in monthly case manager meetings

Experience: Minimum of one year of clinical experience preferred
