Job Title: RN CSP Clinical Assessor

Position: Registered Nurse (RN) for Caregiver Support Program

Job Type: Full Time

Participate in face-to-face and virtual contact with the caregiver and Veteran to assess function and the need for personal care services.
Conduct in-home visits to Veterans and Family Caregivers.
Perform comprehensive assessments, including the Veteran Functional Assessment Instrument (VFAI) and other necessary assessments.
Provide ongoing care management to Veterans and Family Caregivers enrolled in the program.
Coordinate and integrate findings from needs assessments, inter-rater reliability assessments, and program evaluations into learning activities.
Assist in coordinating events such as the annual National Family Caregivers Month, educational sessions, caregiver recognition events, and outreach.

Experience: Five years of experience with case management and clinical assessments preferred

Education: Graduate of a school of professional nursing

License & Certification:

Current, full, active, and unrestricted registration as a graduate professional nurse
Basic Life Support (BLS) certification including AED from American Heart Association or Military Treatment Training Network

Schedule:

COVID-19 Requirements: Participation in seasonal influenza vaccination program required
