Job Title: RN CSP Clinical Assessor

Position: Registered Nurse (RN) for Caregiver Support Program

Job Type: Full Time

Pay: Competitive salary, regular salary increases, potential for performance awards

Shift: Monday through Friday, 8:00 AM to 4:30 PM

Job Responsibilities

  • Participate in face-to-face and virtual contact with the caregiver and Veteran to assess function and the need for personal care services.
  • Conduct in-home visits to Veterans and Family Caregivers.
  • Perform comprehensive assessments, including the Veteran Functional Assessment Instrument (VFAI) and other necessary assessments.
  • Provide ongoing care management to Veterans and Family Caregivers enrolled in the program.
  • Coordinate and integrate findings from needs assessments, inter-rater reliability assessments, and program evaluations into learning activities.
  • Assist in coordinating events such as the annual National Family Caregivers Month, educational sessions, caregiver recognition events, and outreach.

Required Skills

Core Skills

  • Registered Nurse (RN) license
  • BLS certification

Preferred Skills

  • Five years experience with case management and clinical assessments
  • Experience completing home visits
  • Specialized experience with caregivers of Veterans
  • Bachelor's or Master's in Nursing (BSN or MSN)

Requirements

Experience: Five years of experience with case management and clinical assessments preferred

Education: Graduate of a school of professional nursing

License & Certification:

  • Current, full, active, and unrestricted registration as a graduate professional nurse
  • Basic Life Support (BLS) certification including AED from American Heart Association or Military Treatment Training Network

Schedule:

  • Monday through Friday, with telework ad hoc as needed

COVID-19 Requirements: Participation in seasonal influenza vaccination program required
