Job Title: RN CSP Clinical Assessor
Position: for the Caregiver Support Program within Care Management and Social Work Service
Job Type: Full Time
Pay: Competitive salary, regular salary increases, potential for performance awards

Job Responsibilities
- Participate in face-to-face and virtual contact with the caregiver and Veteran to assess function and need for personal care services.
- Conduct in-home visits to Veterans and Family Caregivers.
- Perform comprehensive assessments, including the Veteran Functional Assessment Instrument (VFAI) and other assessments as needed.
- Provide ongoing care management to Veterans and Family Caregivers enrolled in the program.
- Coordinate and integrate findings from needs assessments, inter-rater reliability assessments, and program evaluations into learning activities.
- Assist in coordinating events such as National Family Caregivers Month, educational sessions, caregiver recognition events, and outreach.
- Function independently and handle travel to and from patient homes and within the community.

- Experience with case management and clinical assessments
- Experience completing home visits
- Experience with caregivers of Veterans
- Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN)
Requirements
Experience: Preferred 5 years of experience with case management and clinical assessments
Education: Graduate of a school of professional nursing approved by the appropriate accrediting agency
License & Certification:
- Current, full, active, and unrestricted registration as a registered nurse in a US State, Territory or Commonwealth
- Basic Life Support (BLS) certification
