Job Responsibilities
- Supervise the planning, coordination and delivery of the interdisciplinary group interventions.
- Assign patient/family to appropriate staff member(s).
- Participate and provide leadership in the team for regular interdisciplinary group meetings.
- Develop, review and revise the patient/family plan of care.
- Facilitate problem-solving sessions and deliver team support.
- Implement and monitor compliance with policies and procedures.
- Serve as a member of the management team to address problems associated with service delivery and staff development needs.
- Provide suggestions for appropriate additions/changes in policies and procedures.
- Give peer input to enhance and improve the management function.
- Supervise the maintenance of medical records assuring accuracy, completeness and compliance with standards.
- Provide supervision of support and clerical staff.
- Oversee clinical supervision of nursing personnel, Hospice Aides and Homemakers.
- Conduct appropriate supervisory visits for clinical observation of performance.
- Provide information for written performance evaluations.
- Provide leadership and counsel to nursing personnel for staff development.
- Identify need for staff education and assist in the development of in-service programs.
- Assist in education/training programs.
- Participate in orientation of new personnel.
- Perform specific assignments as directed by the Director.
- Regularly assess and reassess the nursing needs of the Hospice patient.
- Provide Hospice nursing services, treatments and preventive procedures.
- Teach, supervise and counsel the Hospice patient and family members.
- Perform specific nursing procedures following doctor's orders.
- Observe confidentiality and safeguard all patient-related information in compliance with HIPAA.
- Communicate to the Director if unable to meet a patient’s need or perform a procedure.
- Participate in on-call system and provide on-call coverage when unavailable for assigned duties.
- Maintain skills and knowledge.
- Participate in QAPI program.
- Other job duties as assigned.

Requirements
Experience: Minimum 2 years of experience as a registered nurse
Education: Graduate from an accredited school of nursing
License & Certification:
- Currently licensed as a registered nurse to practice in the state
