Job Responsibilities

Recommend employment decisions for staff to the director of operations, including hiring, firing, advancing, promoting, or changing status.
Responsible for overall quality of patient care and clinical performance of nurses and aides.
Organize clinical operations for the care center.
Review requests for services and determine patient eligibility/suitability for home care services.
Coordinate patient referrals and admissions with intake.
Oversee scheduling.
Oversee payor verification and precertification requirements.
Review documentation of other staff members and resolve missing, incomplete, and/or untimely documentation issues.
Assist clinicians in establishing immediate and long-term patient goals, setting priorities, and developing Plan of Care (POC).
Regularly evaluate the needs of the patient and revise the plan of care in collaboration with clinicians.
Provide direct patient care on an infrequent basis in times of emergency.
Participate in the on-call process.
Assist with training of staff to ensure quality care, compliance, and fiscal responsibility.
Participate in the investigation and resolution of patient/family/physician concerns.

Required Skills

Core Skills

Clinical RN experience in home health or hospice
Management experience
Current CPR certification

Preferred Skills

Associate or bachelor's degree in nursing
Current RN license specific to the assigned state

Requirements

Experience: One year of clinical RN experience in home health or hospice and one year of RN management experience.

Education: Associate or bachelor's degree in nursing.

License & Certification:
