Job Responsibilities
- Report directly to the Clinical Manager and collaborate with the interdisciplinary team.
- Provide functional guidance to Team RNs, LVNs, PTAs, OTs, COTAs, SLPs, MSWs, RDs, and HHAs.
- Deliver direct patient care and perform assessments to develop treatment plans.
- Maintain strong working relationships with internal and external customers.
- Exercise independent judgment in planning and providing patient care.

- Thorough understanding of home care and hospice services.
- Strong clinical competencies and assessment skills.
- Excellent organizational and collaborative skills.
Preferred Skills
- Experience in home health and/or hospice settings.
- Leadership and negotiation skills.
- Ability to work with computerized documentation tools.
Requirements
Experience: One year of professional experience in an acute or similar setting required; one year in home health/hospice preferred; five years of direct patient care experience required.
Education: RN graduate from an accredited curriculum; Bachelor's in Nursing or a health-related field preferred.
License & Certification:
- RN-Registered Nurse of California
- Valid Driver's License Class C
- BLS-Basic Life Support Healthcare Provider
Schedule:
- Full Time, Monday - Friday, Variable, Rotating Weekends
