Provide overall management of patients and direct care according to visit productivity standards.
Plan, organize, and direct all patient care services for the assigned caseload.
Coordinate the interdisciplinary team to implement established plans of patient care.
Ensure effective management of visit utilization and control of expenses.
Assist in agency-wide orientation and competency evaluations.
Consult clinical staff related to specific expertise.

Ability to function independently and as a member of an interdisciplinary team
Independent nursing judgment in planning and providing patient care
Knowledge of patho-physiology and treatment protocols for common home care diagnoses
Basic safety and infection control principles
Demonstrated clinical assessment skills
Effective written and verbal communication skills
Understanding of palliative care for terminally ill patients

Experience: 2 years of recent relevant experience

Education: Graduate of an accredited School of Nursing
