Job Title: RN Case Manager, Home Health
Position: RN graduate from an accredited curriculum, with potential functional guidance to Team members.
Job Type: Full Time
Pay: $68.91 to $92.60 / hour
Shift: Days, Monday - Friday, Variable

Job Responsibilities
- Provide functional guidance to Team RNs, LVNs, PTAs, OTs, COTAs, SLPs, MSWs, RDs and HHAs.
- Maintain all required licensure and certifications.
- Manage and administer hazardous drugs per NIOSH and USP 800 guidelines.
- Demonstrate clinical competency and exceed testing levels regularly.
- Form harmonious working relationships with internal and external customers.
- Plan and provide patient care using independent sound judgment.

Requirements
Experience: One year of professional experience in an acute or similar setting required; one year of experience in home health and/or hospice preferred; five direct patient care experience within the past five years required.
Education: RN graduate from an accredited curriculum, Bachelor's in Nursing or health-related field preferred.
License & Certification:
- RN-Registered Nurse of California
- Valid Drivers License Class C
- Automobile Insurance
- BLS-Basic Life Support Healthcare Provider
Schedule:
- Rotating Weekends
- Full-time, 40 hours per week
