Job Responsibilities

Promote patient-centered care by coordinating the plan of care for the patient stay.
Manage the length of stay and ensure appropriate resource management.
Develop a safe discharge plan in collaboration with the multidisciplinary team.
Facilitate the progression and transition of care using established criteria.
Provide case management services for both inpatient and observation patients as assigned.
Identify patients at risk for adverse outcomes during the transition between care levels.
Perform comprehensive assessments of psychosocial, medical, and discharge needs.
Reassess the patient’s clinical condition as indicated.
Develop strategies to mitigate readmission risks, including patient education.
Coordinate the plan of care and drive the discharge plan with the healthcare team.
Partner with Social Services to ensure appropriate post-acute medical needs.
Assume responsibility for timely referrals to Social Services when risks are identified.
Involve patients and families in identifying and clarifying needs and expectations.
Evaluate the progression of care using evidence-based tools throughout the care episode.
Make appropriate referrals for recurring patients and those with chronic disease states.
Facilitate patient throughput with a focus on effective care transition and quality.
Document recommendations, discharge plans, and case management activities.
Align patient needs with available resources for safe discharge/transition.
Act as a liaison between physicians, patients, families, and hospital staff.
Seek ways to control costs without compromising patient safety and quality.
Direct activities to meet the needs of under-resourced patient populations.
Participate in performance improvement activities.
Adhere to established policies and standards of care.

Required Skills

Core Skills

Registered Nurse (RN) license
Case management experience
Clinical nursing experience
Knowledge of regulatory requirements and compliance policies

Preferred Skills

InterQual experience
Bachelor’s Degree in Nursing

Requirements

Experience: 2+ years in case management or 3+ years in clinical nursing

Education: Associate Degree in Nursing or Nursing Diploma required, Bachelor’s Degree in Nursing preferred

License & Certification:

Registered Nurse (RN)
