Shift: Rotating after hours shifts (8's or 12's)

Job Responsibilities
- Provide patient care visits to hospice patients
- Assess patient medical needs
- Liaise with physicians on patient care
- Attend deaths and provide support
- Triage calls from patients and families
- Assist with the admission process to hospice
- Deliver hospice care according to accepted standards of practice
- Integrate all services of hospice into a cohesive support program
- Fulfill RN On Call duties as needed
Required Skills
Core Skills
- Current unrestricted AR RN license
- BLS certification
- Strong assessment skills
- Critical thinking
- High level of problem-solving within a healthcare facility
- Strong documentation and communication skills
- Ability to stay calm in emergencies

- Experience in hospice care
- Home health experience
- Acute care nursing experience
- Computer charting experience
