Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Performs initial assessments, re-assessments, and focused assessments based on patient condition, setting, and population.
Identifies problems based on clinical judgment regarding the patient’s response to health conditions or needs.
Plans and sets measurable and achievable short- and long-range goals in collaboration with patient and family.
Establishes daily goals with the patient and family.
Implements nursing care according to the care plan, ensuring continuity of care.
Ensures patient safety through medication administration and provision of ordered treatments.
Educates patients and families, providing nursing care as required.
Documents care in the healthcare record.
Continuously evaluates both patient’s status and the effectiveness of nursing care, modifying care plans as necessary.
Manages and corrects the sequence, timing, and effectiveness of patient care from admission to discharge.
