Pay: $89,900 - $160,600 annually

Job Responsibilities
- Conduct in-home assessments on health plan members
- Perform annual health assessments, post discharge visits, and visits for complex members
- Review past medical history and systems during assessments
- Conduct medication reviews and evidence-based practice screenings
- Check vitals and perform physical exams including point of care testing as appropriate
- Identify diagnoses for care management and medical management
- Communicate assessment findings to inform member’s primary care provider of potential gaps in care
- Identify needs and opportunities for clinical referrals
- Address urgent and emergent situations appropriately
- Educate members on disease processes, medications, and compliance
Required Skills
Core Skills
- NP license and national certification (ANCC or AANP)
- Experience in a clinical setting
- Proficient with electronic medical records
- Excellent administrative and verbal skills
- Solid communication skills with the geriatric or Medicare population
Preferred Skills
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home health care or home visit experience
- Ability to lift 30-pound bag in and out of car and up and down steps as needed
Requirements
Experience: 1+ years as a Nurse Practitioner in a clinical setting
Education:
License & Certification:
- NP license
- National certification (ANCC or AANP)
- Family, Geriatrics, Adult, Adult-Gerontology Primary or Adult-Gerontology Acute Care certification
Schedule:
