Pay: $89,900 to $160,600 annually

Shift: Flexible scheduling over the 7-day work week, no on-call, no weekends, no holidays

Conduct in-home assessments on health plan members.
Average visit is 45-60 minutes in length.
Perform and complete assessments electronically.
Review past medical history.
Conduct review of systems and medication review.
Perform evidence-based practice screenings and check vitals.
Conduct physical exams that include point of care testing as appropriate.
Identify diagnoses for care management and active medical management.
Formulate lists of current and past medical conditions using clinical knowledge and judgment.
Communicate findings to inform member’s PCP of potential gaps in care.
Identify needs and opportunities to generate clinical referrals.
Identify urgent and emergent situations and intervene appropriately.
Educate members on topics such as disease process, medication, and compliance.

Experience: 1+ years as a NP in a clinical setting

Education:

License & Certification:

NP license and national certification (ANCC or AANP)
Family, Geriatrics, Adult, Adult-Gerontology Primary or Adult-Gerontology Acute Care certification
