Pay: $89,900 to $160,600 annually
Shift: Flexible scheduling over the 7-day work week, no on-call, no weekends, no holidays

Job Responsibilities
- Conduct in-home assessments on health plan members.
- Average visit is 45-60 minutes in length.
- Perform and complete assessments electronically.
- Review past medical history.
- Conduct review of systems and medication review.
- Perform evidence-based practice screenings and check vitals.
- Conduct physical exams that include point of care testing as appropriate.
- Identify diagnoses for care management and active medical management.
- Formulate lists of current and past medical conditions using clinical knowledge and judgment.
- Communicate findings to inform member’s PCP of potential gaps in care.
- Identify needs and opportunities to generate clinical referrals.
- Identify urgent and emergent situations and intervene appropriately.
- Educate members on topics such as disease process, medication, and compliance.
Required Skills
Core Skills
- NP license and national certification (ANCC or AANP)
- 1+ years of experience as a NP in a clinical setting
- Proficient with electronic medical records
- Ability to work independently
- Detail-oriented and reliable
Preferred Skills
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home health care or home visit experience

- NP license and national certification (ANCC or AANP)
- Family, Geriatrics, Adult, Adult-Gerontology Primary or Adult-Gerontology Acute Care certification
