Position: HouseCalls Statewide Traveler - Full Time

Job Type: Full Time

Pay: $89,900 to $160,600 annually

Shift: Flexible scheduling over the 7-day work week, no on-call, no weekends, no holidays

Job Responsibilities

  • Conduct in-home assessments on health plan members.
  • Average visit is 45-60 minutes in length.
  • Perform and complete assessments electronically.
  • Review past medical history.
  • Conduct review of systems and medication review.
  • Perform evidence-based practice screenings and check vitals.
  • Conduct physical exams that include point of care testing as appropriate.
  • Identify diagnoses for care management and active medical management.
  • Formulate lists of current and past medical conditions using clinical knowledge and judgment.
  • Communicate findings to inform member’s PCP of potential gaps in care.
  • Identify needs and opportunities to generate clinical referrals.
  • Identify urgent and emergent situations and intervene appropriately.
  • Educate members on topics such as disease process, medication, and compliance.

Required Skills

Core Skills

  • NP license and national certification (ANCC or AANP)
  • 1+ years of experience as a NP in a clinical setting
  • Proficient with electronic medical records
  • Ability to work independently
  • Detail-oriented and reliable

Preferred Skills

  • Experience in gerontology, cardiology, internal medicine, or endocrinology
  • Home health care or home visit experience

Education:

License & Certification:

  • NP license and national certification (ANCC or AANP)
  • Family, Geriatrics, Adult, Adult-Gerontology Primary or Adult-Gerontology Acute Care certification
